If you have previously taken your employer to court or to an Employment Tribunal, please send us a written description complete with copies of any Court or Tribunal Papers.
Has your employer previously complained about your conduct or performance or ever disciplined you? If so, please give full details in a separate document including details of any disciplinary action taken against you
Have you attended any disciplinary hearings? If so, please give full details in a separate document.
If you believe you have been unfairly dismissed or unfairly made redundant please explain in detail on a separate document why you think it was unfair
If you think that you were constructively dismissed (i.e. you resigned or left your employment because your employer had broken the contract with you for example by ill treating you) please give details on a separate document of how you think your employer has broken the contract
If you are claiming compensation for unfair dismissal/redundancy you need to show that you have made efforts to find alternative employment. Shortly before the Tribunal hearing would you please send us details of what efforts you have made and are continuing to make and attach copies of job advertisements, applications for jobs and so on
To enable us to work out what you earned in your last employment please send us your last P60 form, your P45 form and all salary slips relating to the employment in question
If and when you obtain alternative employment please send us salary slips relating to that employment, your letter of appointment, your notice of conditions of the terms of your employment and your contract, if any
